Blood Test Request

For existing Lemala Health patients only

This form is suitable if:

  • You have been advised to arrange blood tests as part of your ongoing care.

  • You would like to arrange one of our standard blood test panels.

Please contact us instead if you are unsure which blood test is appropriate for you or would like to discuss your results or treatment.

What Happens Next?

  • We will review your request.

  • We will send you an invoice for your chosen blood test panel.

  • Once payment has been received, we will email you details of Nationwide Pathology so that you can arrange your blood sample collection directly.

  • Nationwide Pathology will charge a separate fee for taking your blood sample.

  • Your results will be reviewed by a Lemala Health clinician and reported back to you in line with our usual process.
