Do you need a blood test to diagnose menopause?
Blood testing in menopause care can feel so confusing. Some women worry that without blood tests, perimenopause or menopause might be missed or that treatment decisions are not being individualised.
For most women, perimenopause and menopause are diagnosed based on symptoms and not blood tests.
A single blood test often doesn’t reflect where you are in the hormonal transition. This is because hormone levels, particularly in perimenopause, fluctuate naturally from day to day.
This is why routine hormone testing is not always helpful, and is not required for many women.
That said, blood tests can play an important role in certain situations.
Why hormone blood tests can be misleading
During perimenopause, levels of oestrogen and other hormones rise and fall unpredictably.
This means:
A “normal” result does not rule out perimenopause
A “low” result may not reflect your usual levels
Results can lead to confusion rather than clarity
For many women, focusing on symptoms gives a much clearer picture.
When are blood tests helpful in menopause?
Blood tests can play an important role in certain situations.
As menopause specialists, we may consider blood tests when:
Symptoms are unclear or atypical
Menopause is suspected under the age of 45
Considering testosterone treatment
There are relevant medical conditions or risk factors
Assessing overall health (for example cholesterol or blood sugar levels)
Checking for other causes of symptoms, such as thyroid problems, low iron, anaemia, or vitamin deficiencies
Looking beyond hormones
Good menopause care looks beyond hormones alone.
Symptoms such as fatigue, low mood, poor sleep or brain fog are not always purely hormonal.
Blood tests can sometimes help identify other contributing factors that need addressing alongside menopause care.
This is where an individualised approach really matters.
